How the calculator works

The basis for your metabolic age

The metabolic age calculation is built on published research linking each of the five metabolic markers to biological ageing rates. It is not a simple average — each marker has a specific penalty curve calibrated to the clinical thresholds at which biological age acceleration is measurably documented.

The sources the calculator draws from include:

Fasting glucose & HbA1c → biological age acceleration
Levine et al. (2018) — phenotypic age model based on clinical biomarkers including glucose. Aging, 10(4).
Fasting insulin & HOMA-IR → insulin resistance ageing
Matthews et al. (1985) — HOMA model. Insulin resistance is independently associated with accelerated biological ageing across multiple organ systems.
TG:HDL ratio → cardiovascular age & metabolic risk
McLaughlin et al. (2005) — TG:HDL as the most accurate predictor of insulin resistance in a lipid panel. American Journal of Cardiology.
Blood pressure → vascular age
Wills et al. — arterial stiffness and vascular age modelling. Each 10 mmHg increase above 120 systolic independently predicts cardiovascular event risk.
Waist circumference → visceral fat & metabolic age
Carnethon et al. — waist circumference as a stronger predictor of metabolic age acceleration than BMI across all age groups.
Waist-to-hip ratio → central fat distribution
World Health Organization (2008) — waist-to-hip ratio report. A ratio above 0.90 (men) or 0.85 (women) is classified as substantially increased metabolic risk.
Waist-to-height ratio → whole-population risk screening
Ashwell & Gibson — "keep your waist to less than half your height" as a simple boundary value that predicts risk consistently across sexes, ages, and ethnicities.
Neck circumference → upper-body fat & metabolic syndrome
Multiple population studies converge on ≈37–38 cm (men) and ≈33–34 cm (women) as the threshold linked to significantly increased metabolic syndrome risk.
Resting heart rate → cardiovascular & all-cause mortality
Large cohort studies (Whitehall, Framingham, NHANES) consistently show rising mortality risk above 70 bpm, with the sharpest increase at ≥80 bpm.

Fasting blood sugar — core marker

A snapshot of glucose in your blood after an 8-hour fast. It's one of the five NHANES-defined criteria for metabolic health and the most direct measure of how well your body is managing sugar right now.

HDL cholesterol — core marker

"Good" cholesterol that helps clear excess fat from the bloodstream. Low HDL is one of the five official metabolic syndrome criteria and tracks closely with cardiovascular risk.

Triglycerides — core marker

Fat circulating in your blood, driven almost entirely by carbohydrate and alcohol intake. One of the five core metabolic health criteria, and one of the fastest markers to respond to dietary change.

Blood pressure — core marker

The force of blood against your artery walls. Elevated blood pressure is one of the five core metabolic health criteria and a direct measure of cardiovascular strain.

Waist circumference — core marker

A proxy for visceral fat — the fat packed around your organs. It's the fifth core metabolic health criterion and, unlike BMI, tells you where fat is stored, not just how much you carry.

HbA1c — optional upgrade

A 90-day average of your blood sugar, built from how much glucose has stuck to your red blood cells. It reveals the pattern fasting glucose alone misses, including post-meal spikes.

Fasting insulin — optional upgrade

The earliest measurable sign of insulin resistance — it rises years before fasting glucose does. Rarely included on a standard panel, yet widely considered the single most predictive marker of metabolic decline.

HS-CRP — optional upgrade

High-sensitivity C-reactive protein measures systemic inflammation — a silent driver of heart disease and accelerated ageing. A low reading is one of the clearest signs your body isn't under chronic stress.

Waist-to-hip ratio — optional upgrade

Waist divided by hip circumference. It captures fat distribution rather than just total fat — the WHO classifies a ratio above 0.90 (men) or 0.85 (women) as substantially increased metabolic risk.

Waist-to-height ratio — optional upgrade

Waist divided by height. The simplest whole-population risk screen there is — keeping your waist under half your height is linked to lower metabolic risk across virtually every body type and ethnicity studied.

Neck circumference — optional upgrade

A simple tape-measure proxy for upper-body fat. Multiple population studies link a larger neck to significantly increased metabolic syndrome risk, independent of waist size.

Resting heart rate — optional upgrade

Your heart rate at rest. Large cohort studies consistently link resting rates above 80 bpm to higher cardiovascular and all-cause mortality risk, independent of other risk factors.
